Application for OBIA's Return to Work Program

Hello,

Thank you for your interest in the Ontario Brain Injury Association's (OBIA's) Return to Work Program. The goal of this program is to provide those living with the effects of acquired brain injury with tailored education, training and support to improve the return to work process.

This is a FREE program, offered to those living in Ontario. The Return to Work Program offers:

A self-paced online training program tailored to your needs as a person planning to return to work after sustaining a concussion/brain injury
Enhanced Support Services: in collaboration with your employer, access to an Occupational Therapist and Social Worker for up to 6 months of support

Please note: The Return to Work Program does not provide support for job searching, building resume, interview skills or job coaching. If you are unemployed and looking for work, click here to learn more about OBIA’s Job Readiness Program for assistance in this area.

Your application and participation in the Return to Work Program is completely voluntary. This application will ask you to provide information about your age, gender, employment details, the nature of your concussion/brain injury, and other important details. The information from this application will be used to guide clinical decisions and to evaluate the program.

Your responses will be strictly confidential and only reviewed by OBIA staff. No identifiable information will be shared without your expressed permission. Data may be reviewed and presented externally. All information would be deidentified meaning your name would not be connected to any of the data presented. Confidentiality will be maintained throughout and after the program concludes. You have the right to withdraw your consent (from data collection or from program participation) at any time without penalty.
